Do you or a loved one aged 65 or older need assistance with living at home?

The Social Services Department of Self-Help for the Elderly is dedicated to helping seniors in San Francisco maintain their independence through our unique program, the Multipurpose Senior Services Program (MSSP).

The Multipurpose Senior Services Program (MSSP) offers free case management and supportive services to frail Medi-Cal participants who are 65 years of age or older and are eligible for nursing facility placement but wish to remain in their homes. We provide professional assessments, connections to existing community resources, and care coordination to help prevent or delay the need for institutional placement.

Services Include:

  • Professional Assessments: Personalized care plans to meet your unique needs.
  • Community Resource Linkage: Access to transportation, meal delivery, home modifications, and more.
  • Care Coordination: Ongoing support to keep you safe and comfortable at home.

Who Qualifies?

  • Aged 65 years and older
  • Medi-Cal individuals
  • Nursing Facility (NF) level of care
  • Reside in San Francisco
